Description
The Tomed SomnoGuard® SP Soft is a two-piece mandibular advancement device designed to reduce snoring and help manage mild to moderate OSA. It works by gently holding the lower jaw forward during sleep to keep the airway open, using side connectors that allow millimetre-precise adjustment from -3mm to approximately 10mm.
Each jaw tray features a stable outer shell with a thermoplastic inner layer that softens in hot water and moulds to your teeth and bite for a secure, custom fit using the boil-and-bite method. For best results, we strongly recommend having the device fitted by a healthcare professional (such as a dentist, ENT specialist, or sleep physician), although it can be fitted at home.
The Tomed SomnoGuard® SP Soft is suitable for small to medium-sized jaws and is also recommended for individuals with a protruding lower jaw. It allows mouth breathing and slight mouth opening during use.
The material contains no allergens, latex, silicone, BPA, or phthalates. The typical lifespan of the product is 1½ years, though heavy jaw movement or teeth grinding (bruxism) may shorten this.
Please note: Consult your dentist if you have concerns about your teeth or jaw alignment before using, or your doctor if you have concerns about your snoring or OSA.

Snoring and mild to moderate OSA often occur when the airway becomes narrowed or blocked during sleep. This can happen when the muscles in the throat relax, causing the tongue and soft tissues to fall back and restrict airflow.
The Tomed SomnoGuard® SP Soft is worn at night and uses two side connectors to hold the lower jaw slightly forward. By advancing the jaw, the device opens the upper airway and stabilises the soft tissues, reducing their vibration and fluttering, which are responsible for snoring sounds.
The adjustable jaw advancement (from -3mm to approximately 10mm) allows you to find the optimal position to keep your airway open. This helps reduce snoring and may improve breathing during sleep by limiting airway obstruction associated with mild to moderate OSA.
Please note: Due to hygiene reasons, fitted or used devices cannot be returned, so refunds aren’t possible if fitted incorrectly.
For best results, we strongly recommend having the device fitted by a healthcare professional (such as a dentist, ENT specialist, or sleep physician).
However, we understand some users may choose to fit the device themselves. If you do, please carefully follow the detailed instructions included with your product.
The Tomed SomnoGuard® SP Soft uses the boil-and-bite method. Each jaw tray consists of a stable outer shell and a thermoplastic inner layer that softens in hot water and moulds to your bite when cooled.
The fitting process takes about 20 minutes and involves the following steps:
- Heat the upper and lower jaw trays in boiling water for approximately 3½ minutes.
- Carefully bite into the softened trays so the material adapts to your individual tooth shape.
- Cool both trays in cold water so the material hardens and retains your dental impressions. The fitting process can be repeated if needed (e.g., if the trays do not fit optimally).
- Attach the side connectors and adjust the lower jaw advancement to the desired position.
The lower jaw advancement is adjusted using the included side connectors, which come in a range of lengths. These connect the upper and lower trays laterally, allowing millimetre-precise forward positioning of the lower jaw, from -3mm to approximately 10mm. Greater advancement helps open the airway more effectively, which may help reduce snoring and nighttime breathing pauses.

Expensive but Works Quite Well
The MAD was recommended to me by my consultant as he reckoned that an operation was quite painful and wasn’t guaranteed to work or, may only last 6 months. He advised against getting a very cheap device as you get what you pay for. I thought this device was a little on the expensive side but was prepared to take a gamble rather than being poked in the ribs every night. It took quite a bit of preparation and careful reading of the instructions to get the fitment correct but its worth it in the long run. The good thing about this device is that you can adjust it if you were ever unfortunate to lose one or more teeth etc. After a couple of months of using this product I’m still trying to determine the correct adjustment to stop snoring completely but on a more positive note, my ribs are no longer bruised. It’s not easy to remove and replace the adjustment bars but there are a good varying length of bars to chose from until you get the adjustment right. It may seem a little uncomfortable when first using the MAD but you get used to it over a period of time and sometimes you forget you are wearing it.I don’t know if any other users have experienced the same problem as myself but sometimes I may wake up in the early hours with a dry mouth. If this happens I go to the bathroom and give the MAD a rinse under the cold tap. It generally works well then until time to wake up.If I had to estimate how often it worked I’d hazard a guess at 9 nights out of 10
In control of snoring
Purchased the Somnoguard SP Soft 9 months ago but kept putting off the fitting (it’s a male thing)! Partner started threatening me with excommunication if I did not act promptly last week (for obvious reasons). Fitting was easy, needed a couple of minor adjustments over the following few days (plastic was rubbing against gums). Minor discomfort for first few nights but already sleeping much more soundly and no Apnoea since fitting. Partner also sleeping better and very happy with results.
Brilliant!
I bought this for my husband who snores. We now both get a whole nights sleep. He used to snort loudly in between snoring, wake me up (and himself) then roll over and continue snoring whilst I’d lay awake listening to him for hours. With this device he is quiet and the odd night when he forgets to wear it sure enough he is snoring loudly again. Would highly recommend for anyone to buy this as a present for their partner if they snore.
snoring relief
I have tried everything’s not the market, literally everything, including surgery in Harley St without success. I am not overweight, 6ft and 83kgs, have a good diet and get exercise but snore like a raging bull. Until now that is. This mouth guard actually works! It takes a few goes to get the right fitting but start off with a comfortable setting then increase the length of strip you use until the snoring stops. Still a bit of discomfort in the morning but this passes within 30 mins or so. I use Snorelab on an iPhone to record and measure my snoring during the night and my score has dropped from an average of 40 down to 3!I don’t have to worry about waking my partner as I have been single for a few years now, my snoring was a major factor in our marriage collapsing. Since then I had not pursued a relationship for fear my snoring would put someone off, but now I am thinking about trying again as I stand a chance of not getting thumped in the chest in the middle of the night, at least not because I was snoring! 🙂
Review Title
I have been using the Tomed SomnoGuard SP Soft for about five to six weeks now. I have found it very helpful. According to my wife, my snoring and sleep apnoea have stopped completely and I certainly feel less sleepy during the day and have more energy. It is a bit bulky in the mouth and takes a few nights to get used to it but essentially I was sleeping through the night and not not noticing it until waking by the third or fourth night. Occasionally the lower mandibular part slips off the teeth and needs repositioning despite my having remoulded it. The initial moulding process for both upper and lower parts is straightforward but I needed a couple of attempts to gauge the correct side arm lengths to position the lower jaw relative to the upper. The moulding is soft and comfortable despite its bulk in the mouth. Cleaning it is also straightforward. It is difficult to sense how long it will last – there is very little if any wear at the moment and I would be surprised if it did not last several months to a year. There are spare side arms. Provided it does last about a year it it is reasonable value and will be more than worth the expense.
