The Tomed SomnoGuard® AP 2 is a two-piece mandibular advancement device designed to reduce snoring and help manage mild to moderate obstructive sleep apnoea (OSA). It works by gently holding the lower jaw forward during sleep to keep the airway open, using a central adjustment mechanism that allows millimetre-precise advancement from 0 to approximately 10mm.

It’s recommended for neutral bites and medium to large-sized jaws. Thanks to the lateral mobility of the lower jaw, it’s also suitable for individuals who grind their teeth, though it’s not a traditional night guard for teeth grinding (bruxism).

Description

The Tomed SomnoGuard® AP 2 is a two-piece mandibular advancement device designed to reduce snoring and help manage mild to moderate OSA. It works by gently holding the lower jaw forward during sleep to keep the airway open, using a central adjustment mechanism that allows millimetre-precise advancement from 0 to approximately 10mm.

Each jaw tray features a sturdy outer shell with a thermoplastic inner layer that softens in hot water and moulds to your teeth and bite using the boil-and-bite method. For best results, we strongly recommend having the device fitted and adjusted by a healthcare professional (such as a dentist, ENT specialist, or sleep physician), although it can be fitted at home.

The material contains no allergens, latex, silicone, BPA, or phthalates. The typical lifespan of the product is 1½ years, though heavy jaw movement or bruxism may shorten this.

How does the Tomed SomnoGuard® AP 2 work?

Snoring and mild to moderate OSA often occur when the airway narrows or becomes blocked during sleep. This can be caused by relaxed muscles in the throat, allowing the tongue and surrounding soft tissues to fall back and restrict airflow.

The Tomed SomnoGuard® AP 2 is designed to gently reposition the lower jaw and tongue slightly forward during sleep. This forward shift is adjusted with millimetre precision using a small stainless steel screw located between the upper and lower jaw trays.

By helping to open the upper airway and stabilise soft tissues like the soft palate and uvula, the Tomed SomnoGuard® AP 2 can reduce the tissue vibrations that cause snoring and support more consistent nighttime breathing in people with mild to moderate OSA.

How is the Tomed SomnoGuard® AP 2 fitted?

The Tomed SomnoGuard® AP 2 uses the boil-and-bite method. Each jaw tray consists of a stable outer shell and a thermoplastic inner layer that softens in hot water and moulds to your bite when cooled.

The fitting process takes about 20 minutes and involves the following steps:

  1. Heat the upper and lower jaw trays in boiling water for approximately 3½ minutes.
  2. Carefully bite into the softened trays so the material adapts to your individual tooth shape.
  3. Cool both trays in cold water so the material hardens and retains your dental impressions. The fitting process can be repeated if needed (e.g., if the trays do not fit optimally).
  4. Adjust the advancement of the lower jaw by turning the small stainless steel screw.

The upper and lower jaw trays of the Tomed SomnoGuard® AP 2 are connected by a small stainless steel screw. A guide groove on the underside of the upper tray holds the screw head in place and allows smooth, controlled movement. By turning the screw, you can continuously adjust the lower jaw advancement from 0 to approximately 10mm.

Millimetre markings on the sides of the device help you track the advancement precisely. Increasing the advancement moves the lower jaw further forward, which can help open the upper airway more effectively, potentially reducing snoring and breathing interruptions caused by obstructive sleep apnoea.

All good so far!

Having worn a number of the more basic SomnoGuard 3s previously I’ve realised that this is much more suited. Gives a good jaw opening as well as advancement, and I was so glad to hear my fiancee saying how much quieter I am now! My dentist more recently said that bruxism is quite bad, so this hinged product with good swivel room is better for protecting that too.

Seems to work well

It seems to work well in terms of stopping most snoring; my partner is definitely getting better sleep. It’s can be a little uncomfortable sometimes, and takes a little time to get used to, but I’m finding this improves with time. My jaw and teeth do feel a little strained in the morning, but it fades as the day progresses. My main concern with the SomnoGuard was that I have a crown on one of my smaller molar teeth, and I’ve read that these devices don’t work well with crowns, but this hasn’t been a problem. However, if you did have a crown on any of your front six teeth I can imagine it would be quite different, as these take the brunt of the effort of keeping the lower jaw forward. Overall, I’m please with this purchase, and happy that I spent a little more on the version that allows some movement laterally.

My safety gadget

This is my third SomnoGuard and I couldn’t manage without it. I suffer from sleep apnoea and snoring. My wife was being badly disturbed by the snoring. I eventually purchased my own SomnoGuard when it first came out. I wouldn’t be without it. Together with my Nozovent our nights sleep is not disturbed. I would recommend this to every sufferer. I rated it 9 out of ten because it doesn’t stop me snoring when I’ve had one or two drinks which is no fault of the item!
